Registration Agreement

I hereby give consent for my child/self to participate in training programs/activities or usage of facility provided by CBA. I understand and acknowledge that injuries, permanent disability, or death can occur during participation in a sport such as baseball. I will abide by all rules listed on the website, by the coaches, and posted regarding usage of helmets, net barrier and other safety measures.  I hereby release and hold harmless, covenant not to sue and forever discharge CBA, its owners, and its agents, employees, volunteers, coaches, officials, and facility/training affiliates for any and all claims for damages resulting from personal injuries, illness or otherwise to myself or my child which may arise from participating in any CBA training activity or usage whether or not caused by the negligence (active or passive) of CBA.  In the event that I am unable to communicate my own desire, or I cannot be reached during an emergency involving my child/self, I hereby give consent to emergency medical treatment deemed necessary by a licensed medical provider/hospital.

CBA reserves the right, at its sole discretion, to withdraw any player whose influence or actions are deemed unsatisfactory to the program/training environment. If this occurs, no reduction or return fee, or any part thereof, will be made.  The signature of the legal guardian/parent has the authority over all other biological/custodial parents/guardians to allow child to participant and execute this waiver. If I am signing this form as the parent or legal guardian of the above named minor, I hereby represent I have legal authority to sign this form as the parent or legal guardian of the above named minor.
